Outpatient Hospice / Medical Social Worker - Contract Assignment

Location: Albany,OR
Assignment: 13-Week Contract - Fully Renewable
Guaranteed Hours: 40 Hours/Week
Schedule: Monday-Friday | 8:00 AM-4:30 PM
Weekends: Not regularly required
EMR: EPIC
Setting: Home Health / Mobile Field Operations

Job Summary

We are seeking a compassionate and experienced Medical Social Worker to provide psychosocial support and care coordination to patients and families receiving hospice services throughout regional Oregon communities.

This is a field-based position with patient visits taking place in homes, skilled nursing facilities, assisted living facilities, and adult foster homes. The ideal candidate will be comfortable working independently while collaborating with an interdisciplinary care team to provide compassionate, patient-centered support.

Key Responsibilities

• Provide psychosocial assessments, counseling, crisis intervention, and emotional support to patients and families.
• Provide end-of-life support and bereavement counseling as appropriate.
• Collaborate with nurses, physicians, spiritual care providers, volunteers, and other members of the interdisciplinary team.
• Connect patients and families with appropriate community resources and support services.
• Advocate for patients and assist families with navigating healthcare and social service systems.
• Document patient assessments, visits, interventions, and care plans accurately in EPIC .
• Maintain compliance with applicable healthcare, documentation, and professional standards.

Requirements

• Active, unrestricted Oregon LMSW or CSWA license required at the time of submission.
• LCSW is highly preferred.
• Minimum 1 year of professional social work experience .
• Hospital, medical, hospice, palliative care, or healthcare social work experience strongly preferred.
• Current BLS certification required.
• Valid driver's license and reliable personal vehicle required.
• Active auto insurance meeting applicable state requirements.
• Ability to travel throughout the assigned service area for patient visits.
• Must be available for scheduled holiday coverage when required.
• US Citizens or candidates with active permanent US work authorization only.
• No visa sponsorship or remote work accommodations available.
